Autism Acceptance March 2025: Transitions Our Transitions service delivers a specialist holistic service that improves a young person’s transition readiness, when leaving school to secure socially inclusive and sustainable outcomes in their local communities. The service fully involves young people and their families to make decisions about their life after school, with regard to; Education and work Community inclusion Health and wellbeing What support is provided: This can be delivered through a variety of intervention levels; Transition advice workshops Personal development support One to one transition readiness support How to get involved: Our Transitions Service is available in the Western, Northern, South Eastern, and Belfast Trust areas supporting young people aged 16-20 years to make appropriate choices when moving on from school to further or higher education, training or employment ensuring they achieve their maximum potential. We support young people with disabilities and healthcare needs, autism and brain injury. *The criteria for admission and length of service is dependent upon the contracts we hold in these areas.
