Latest News Transitions Service is Shortlisted for a Children & Young People Now Award We are delighted that our Transitions Service has been shortlisted for The Advice and Guidance Category in the Children & Young People Now Awards. The Awards highlight the very best practice in work with children, young people and families. In a year of turmoil and unprecedented challenges, the Awards recognise innovation and impact across the sector. Cedar’s Transitions Service supports young disabled people aged 16-20 years to make appropriate choices when moving on from school to Further or Higher Education, training or employment ensuring they achieve their maximum potential. The overall aim is to deliver a specialist holistic service that improves a young person’s transition readiness, when leaving school to secure socially inclusive and sustainable outcomes in their local communities. The aims of the Transition Service are to: Support young people and their families make decisions about their life after school. Explore a range of person-centered post school options including education & employment, community inclusion and health & wellbeing. Be an advocate for the young person promoting their abilities. Provide support by developing an action plan with you identifying the goals for the service which will be reviewed with you on a regular basis. Support the transition into your chosen provision and maintain contact and track your progress for up to 6 months where you will then exit the service to continue in your chosen activity independently. Watch Ross`s Transition Service and how it has helped him through the transition from School to Further Education How to get involved: Cedar’s Transitions Service is available in all 5 HSC Trusts across Northern Ireland supporting young people aged 16-20 years to make appropriate choices when moving on from school to Further or Higher Education, training or employment ensuring they achieve their maximum potential. We support young people with disabilities & healthcare needs, autism and brain injury*. The criteria for admission and length of service is dependent upon the contracts we hold in these areas.
